The Shoulder requires an assessment prior to admission into
   any of our treatment services.

  • Those who are local to Baldwin County and Mobile may schedule an assessment at one of
The Shoulder's two offices. An assessment requires a non-refundable fee of $50.

  • Those who are not local to the Mobile area should contact the mental health department in their
county, and schedule a substance abuse assessment. Ask the facility to send the
completed assessment to The Shoulder by FAX to determine whether we are the right
placement for treatment. FAX number: 251-626-2388.

For more information, or to schedule an assessment, please call:
             Daphne office: 251-626-2199

  • ONLY potential clients who are incarcerated, or who live in a county in which there is no mental
health facility available, may download the following screening material. If neither of these
exceptions apply, the paperwork will not be accepted for potential admission.
